However, if you have a diagnosed chronic health condition and require coordinated, ongoing care, you may be eligible for Chronic Disease Management (CDM) through Medicare. In this case, your GP must complete a CDM referral form to access subsidised Exercise Physiology treatment under a Team Care Arrangement (TCA). This allows eligible patients to receive a set number of rebated sessions per calendar year.

Fees may vary based on referral pathways like Medicare CDM, NDIS, DVA, Workers Compensation, or other funding schemes. Private health rebates may be available depending on your cover and provider.
